Due to the Coronavirus pandemic, the UIL has amended some of the requirements for students to participate in athletics. Please read below.
“For the 2020-2021 school year only, any participant in athletics or marching band, in any grade level, who has not previously completed a Pre-participation Physical Evaluation- Physical Examination, and been cleared for participation, will be required to complete the Pre-Participation Physical Evaluation-Medical History Form as noted below, AS WELL AS a Pre-Participation Evaluation-Physical Examination prior to participation in any UIL practices, games, performances, or matches.
ALL students are required to fill out a Pre-Participation Physical Evaluation- Medical History Form annually. This helps determine if your student is required to receive a Pre-Participation Physical Evaluation- Physical Examination due to recent injury or illness. The athletic trainer will decide if the Physical Examination is required depending on your answers.
ALL student-athletes must print, fill out, and turn in the following: Pre-Participation Physical Evaluation- Medical History, Concussion Acknowledgement Form, Sudden Cardiac Arrest Awareness Form, Parent/Student Steroid Agreement Form, Acknowledgement of Rules Form and PSPCISD Medical Release Form.”
